You can continue by completing the online form below. Once we receive your online complaint we will send an acknowledgement email, except for anonymous complaints.

All complaints lodged against staff or services delivered by Care1 will be managed with confidentiality.

Complainants are more than welcome to remain anonymous, however, we may not be able to provide any update to your request for feedback or the complaint handling process.

You can also give feedback to the NDIS Commission or iCare directly. They receive feedback or complaints about services that you are not happy with.
